Be Well Live Well is a program aimed to support people living with, or at risk of, chronic health conditions to make long-term lifestyle changes to positively impact your health and wellbeing.
This unique program uses evidence-based behaviour change techniques to help you create lifelong healthy habits. Information and skills sessions are delivered by health professionals including a physiotherapist, occupational therapist, dietitian, nurse, exercise physiologist and supported by our community connector.
The group sessions run for 9 x 2-hour session and are delivered in a supportive environment which includes a mix of education and structured exercise or physical activity sessions tailored to your needs.
You will learn new skills to help overcome barriers and take better care of your health.
The program supports you to:
- build confidence and knowledge to make healthier choices
- have a greater sense of control over your health
- connect with others in a friendly setting.
More information
Anyone over 18 with, or at risk of developing, a chronic condition. Carers and family members are welcome to attend.
A one-to-one health consultation before and after the program is required. You may also need to be assessed by an Exercise Physiologist or get medical clearance from your GP prior to ensure you can exercise safely.
The program runs on Mondays from 1-3pm at our Bell Street site.
