Community Connections is here to support you to set your goals and get involved in social groups and activities that match your interests.
Community Connections

Supporting you to connect or stay connected with your community

Being involved in your community is good for your health and wellbeing. Our Community Connections program can provide personalised support to help you develop goals and engage in social groups and activities. 

We can support you to regain your independence and the confidence to connect with others in the community. 

We do this in a range of ways:

  1. Individual support Our Community Connections program supports eligible adults aged under 65 to connect with their community. We can work with you one-to-one to develop your goals and assist you to build meaningful social connections through community activities and access to Holstep Health programs.
  2. Workshops and groups We run groups and workshops designed to build your capacity, confidence, and community connections as you take part in meaningful and enjoyable activities.  
  3. Health and Wellbeing Talks We run a variety of learning sessions across the year. They give you information about various topics to help you have an active and healthy lifestyle. We update our programs based on community interest to ensure we cover topics that are engaging, up-to-date, and of interest to you.   

Individual support

Individual and personalised sessions with our Community Connections team.  

We offer an initial appointment and one-to one sessions to help you feel more confident to connect with others in the community. We will work with you to develop your goals and support you to create meaningful connections.

Support may include:

  • giving you information about community resources and ways to increase your social connections.
  • help to access different community activities or services as you work towards achieving your goals. This might include access to community groups, services, recreation opportunities, support services, volunteering opportunities or Holstep Health services.
  • connecting you with our Community Connections social groups and/or seminars. Click the links below to learn more about these options.

Who is this for?

Adults aged 18 to 64 who do not receive other government funding (such as NDIS) and who live in Merri-bek or Hume.

What do I need to do?

You do not need a referral to use this service. You can contact us to join the program by: 

  • calling 8319 7408 and leaving a message with your contact number. We check our voicemail regularly and will get back to you.
  • emailing us at [email protected], or
  • calling our Service Access team on 1300 637 744.
For referrers/Health professionals:

To refer a client, please complete our referral and consent forms and return them via the email below.

How much will it cost?

Some of our programs are free and some have a small cost. Contact us for more information.

Where can I do this?

Support is flexible and can be provided:

  • over the phone
  • in your home 
  • in the community
  • at a Holstep Health site.

Workshops and groups

We run groups and workshops designed to build your capacity, confidence and community connections as you take part in meaningful and enjoyable activities.

Groups are developed based on community feedback and designed with community where possible.

We offer a range of activities – for example Mindfulness, Cooking, Beyond Boardgames, Social café group.  Contact us today to find out how to join in.  If you don’t see something that sparks your interest? Health and Wellbeing Talks

Throughout the year, we run learning sessions on a range of topics related to health and wellbeing. These aim to increase your knowledge and confidence and help you connect with your local community.

We develop the sessions based on community feedback and in collaboration with the community where possible.

Who is this for?

Adults aged over 18 and their families.

Some sessions are open to the wider community. We can also develop and run seminars for established groups.

What do I need to do?

You do not need a referral to attend. Please contact us if you are interested in attending. You can register to attend a specific session via our event flyers.
