Services for those under 65 years old

  • These sites serve surrounding suburbs as well
  • If you can make it to one of these sites regularly, we take referrals from outside Merri Bek local government area - except for Children's Health Services
  • Many services do not require a referral from a health professional. You can refer yourself.
  • Please check our services to find out which ones to need a referral from a health professional
  • If you are a health professional and you can refer to Brunswick, Coburg, Fawkner or Glenroy using this form. It can be faxed back to 03 9495 2599 or emailed to [email protected]
  • The referral form is not for Children's Health Services (Audiology, Dietetics, Occupational Therapy, Speech Pathology). Please contact 1300 637 744 to discuss availability and eligibility.

Our Referral and Intake team for these locations can provide more information about service availability and eligibility.

Services for those over 65 years old

We provide services for those over 65 years old at all our metropolitan Melbourne locations. You must be registered with the Commonwealth Government’s My Aged Care to use the services - even if you have a referral from your treating health professional. There are a few services where this is not needed - information is shown below.

My Aged Care (MAC) registration and assessment.

You can register for a My Aged Care online or phone them on 1800 200 422. The process involves you providing your details to MAC and they will assess your needs.

Referral Codes and why we need them.

You need a referral code to pay for the service you need (e.g. podiatry, occupational therapy) from your MAC funding. You will need to provide your referral code to us to start using our services. You can find your referral codes if you log into your My Aged Care account.

  • 1300 637 744 for our services based in Brunswick, Coburg, Fawkner and Glenroy.

Support at Home (formerly Home Care Packages)

Well@Home – Allied Health and Nursing for clients on a Support at Home package

At Holstep Health our Well@Home team can also provide Allied Health services to those who are already on a Support at Home package.

Chronic pain service

If you’re living with persistent (chronic) pain, you’re not alone—and there is real hope for recovery. Pain is real, but it doesn’t always mean your body is damaged. Often, chronic pain is the result of the brain misinterpreting safe signals from the body as if they were dangerous.

Our chronic pain service offers individual and group support to help you understand pain, to learn practical skills, and build the confidence to live your best life.

Day rehabilitation

The North East Recovery and Support Program (NeRASP) supports clients to take steps toward the life they want to live. This might mean reducing or stopping substance use, improving mental wellbeing, reconnecting with parenting, or returning to study or work.

The program runs on Tuesdays and Thursdays, 10am-3pm for eight weeks. Clients can then get individual counselling on Monday, Wednesday, and Friday for further support. The groups run in school terms with four groups per year.

Diabetes support services

One-on-one support and group education to help you manage your diabetes and health and live well.

Anyone aged 18 and over is eligible that:

  • Any type of diabetes including Type 1, Latent Autoimmune Diabetes and Type 2 diabetes mellitus

You are not eligible if you:

  • are under 18 years of age – refer to Royal Children’s Hospital on (03) 9345 5522
  • need support with management of gestational diabetes or are pregnant and has diabetes – refer to Royal Women’s Hospital on (03) 8345 2000

Dietetics

Advice and support from a dietitian on nutrition and how to make better food choices to improve your health based on your needs. We have one-on-one consultations and group education programs.

Referrals for those aged 64 and under

Referrals for those 65 years and over

  • You will need a referral from My Aged Care who will carry out an assessment.
  • A doctor or health professional can refer you to My Aged Care. 

You are not eligible if you:

  • have an eating disorder – refer client to Eating Disorders Foundation on 1300 550 236
  • lives in a Residential Aged Care service
  • has Gestational Diabetes or needs support with management of diabetes during pregnancy – refer to Royal Women’s Hospital (03) 8345 2000

Exercise physiology

Exercise assessment, prescription and health education to help with daily living, disease management, general health and integration into the community.

Referrals for those 65 years and over

  • You will need a referral from My Aged Care who will carry out an assessment.
  • A doctor or health professional can refer you to My Aged Care. 

A person is ineligible if they live in a nursing home (high level care).

Cost:

GLA:D® – Hip and knee osteoarthritis group

The GLA:D® (Good Living with Arthritis: Denmark) program, is a proven education and exercise program specifically designed for hip and knee osteoarthritis.

LGBTIQA+ health: Alcohol and drug services

Counselling and support service known as QHealth for people who would like to talk about the concerns or impacts of their alcohol and drug use in a confidential, non-judgmental and queer-affirmative space.
