Care coordinators work with you, your GP and other health and community services to help you manage your health and wellbeing.
Care coordination can help you understand the health system and access the support and services you need.
A care coordinator will do a thorough assessment and develop a care plan based on your preferences, health needs and goals.
Services may be offered in-person or via telehealth or telephone.
