Services for those under 65 years old

Services for those over 65 years old
We provide services for those over 65 years old at all our metropolitan Melbourne locations. You must be registered with the Commonwealth Government’s My Aged Care to use the services - even if you have a referral from your treating health professional. There are a few services where this is not needed - information is shown below.My Aged Care (MAC) registration and assessment.
You can register for a My Aged Care online or phone them on 1800 200 422. The process involves you providing your details to MAC and they will assess your needs.Referral Codes and why we need them.
You need a referral code to pay for the service you need (e.g. podiatry, occupational therapy) from your MAC funding. You will need to provide your referral code to us to start using our services. You can find your referral codes if you log into your My Aged Care account.If you already have a referral code and wish to access our services, contact us:

Memory loss and dementia support
If you’re experiencing memory changes or living with dementia, we’re here to help you stay independent and continue doing what matters to you. Our team will work with you and your caregivers to understand what’s happening and offer practical and emotional support tailored to your needs.

Occupational therapy
Assessments and advice in your own home, to help you achieve maximum mobility and comfort at home. This may include installing equipment to help you feel safe and prevent falls. An occupational therapist will visit you at home.
Anyone is eligible who is living at home or in community residential units within 8km of 93 Bell Street, Coburg. Home visits outside of the 8km area must be discussed with our team first.

Referrals for those 65 years and over
- You will need a referral from My Aged Care who will carry out an assessment.
- A doctor or health professional can refer you to My Aged Care.
You are not eligible if you:
- are under 18 and not accompanied by a parent or guardian
- live in a residential aged care facility – refer to a private occupational therapist
- is under Work Cover or TAC – contact OT Australia
- has been discharged from a public hospital in the last 4 weeks and needs equipment related to the discharge – refer to hospital
Physiotherapy
Support to manage injuries and physical problems through home-based exercise programs, water exercises, gentle gym classes and chair-based classes.
Referrals for those aged 64 and under

Referrals for those 65 years and over
- You will need a referral from My Aged Care who will carry out an assessment.
- A doctor or health professional can refer you to My Aged Care.
You are not eligible if you:
- lives in a nursing home (high level care)
- is funded by Work Cover or TAC with a chronic condition and/or complex care needs, that is eligible for subsidised MBS allied health services, via a CDM referral from their GP – refer to private physiotherapy services
Podiatry
Prevention and treatment of foot, ankle and lower leg problems. Information on how to avoid problems and advice on foot care and footwear. A podiatrist can visit you at home, if needed.
Anyone aged 18 and over is eligible that:

is willing to travel to Coburg, Brunswick, Glenroy or Fawkner for centre-based appointments

is a Department of Veteran Affairs client – a referral form from a GP is needed

has an active foot ulcer and does not need tertiary care
Refer directly to the Royal Melbourne Hospital if they have:
- signs of systemic infection, such as feeling unwell or a high temperature
- a chronic wound infection – the infections needs more than two courses of antibiotics
- a wound that has exposed tendon or bone
- peripheral arterial disease

- A doctor or health professional can refer you to My Aged Care.
You are not eligible if you:
- are aged 0-8 who has a foot, lower limb or gait condition that is not related to a skin and nail condition – refer to the Children’s feet and leg clinics
- live in a nursing home (high level care)
Speech pathology
We provide help to diagnose and treat problems related to swallowing and communication like speech and language difficulties, general communication, choking or coughing on food, memory and voice problems.
Anyone aged 18 and over is eligible that:

has a disability and is experiencing a communication and/or swallowing disorder
- is a carer
Referrals for those 65 years and over
- You will need a referral from My Aged Care who will carry out an assessment.
- A doctor or health professional can refer you to My Aged Care.
You are not eligible if you:
- are funded by Work Cover or TAC
- live in a residential aged care facility
- are aged 18-64 years and does not have a disability or is not a carer
- are under 6 years of age
- need intensive therapy services
- are aged 6-18 years and has a diagnosed disability
- has a permanent tracheostomy
- has laryngectomy
Speech Pathology for children
We provide early support to help children build their talking and listening skills.
For children who have not started school (0 – 5 years) with mild to moderate development delays who benefit from with short-medium term therapy needs.
We are not a NDIS therapy provider.
We accept children who live or attend kinder or childcare in the local government areas of Merri-bek and Banyule.
Priority is given to low-income families, and families experiencing hardships.
The 3081 Angels
3081 Angels are a volunteer run, not-for-profit organisation that aims to make sure every child in our community has their basic material needs met.
How can workers refer?
A simple email from a health, welfare, education or community worker will act as a referral. The email must include:
- a brief introduction to the family requesting assistance with material aid
- client name, address and phone details (if permission is given). Angels do not need the client’s details. The worker can make all client contacts themself
- goods required.
Requests for specific goods should include:
- Girl/Boy/Unspecified
- Age
- Clothing/shoe sizes
- Child(ren) age(s) for pram and car seat requests
- Other relevant information or additional needs
- Whether delivery is requested or goods will be collected.
Free delivery is offered within 30 minutes of our base in Heidelberg Heights, (usually on Fridays).
