Health record requests

How to access your health information

To deliver our health services, we need to collect information about the people who use them. If you’re one of our clients, the information in your health record helps us provide you with safe, high-quality, personalised support. 

We manage this information in line with the Health Records Act 2001, and we’re committed to keeping it private and secure. 

You may want to access your health information for various reasons. For example, you might need it to:

  • transfer your care to another provider
  • keep a record of your discussions with healthcare professionals
  • get a letter or report from your healthcare provider
  • share with legal services or advocates
  • take part in the Child Information Sharing Scheme or Family Violence Information Sharing Scheme
  • comply with subpoenas or coroner requests
  • provide information to a representative or guardian.
How to request your information
To request your records, email the Quality team. This ensures your request reaches the right team who can gather the necessary information for you.

Frequently asked questions

How long do you keep my health information?

We keep our records in with our legislative requirements which is typically up to 7 years from the date of last service but may be longer in some cases. If you have any questions please contact our Privacy Officer [email protected].

In line with legislation, we retain adult health records for 7 years from the date of the last service. Children’s records are kept until they turn 25.

How long will it take to receive my records?

We aim to respond to all requests as quickly as possible and will reply to your request within 45 days. If you need all your records, this might take longer than specific requests for part of them.

Can someone else request my records?

In general, we will only release your health information to you or with your consent. We may need to share your records with someone other than you if:

  • they are subpoenaed for legal proceedings 
  • a regulator or governing body requires us to provide them
  • we need to share information to ensure the safety of a child or a person escaping from family violence
  • they are requested by an authorised body or individual, such as the coroner, a parent/guardian of a minor, an advocate or a medical power of attorney.
