Health assessment, advice and support
Our community nurses provide personalised care to help you manage your health and access the services you need. We focus on helping you to stay independent and have the best possible quality of life.
Unlike hospital or clinic-based nurses, community nurses support you where you live. Depending on your needs, we can see you at home or in one of our community clinics.
Our reliable and caring nurses can help you with:
- general health and dementia assessments
- continence advice and access to aids
- help with funding applications for health supports
- health information and lifestyle advice
- referrals to other health professionals, such as dietitians or counsellors.
More information
People 65 years and over (50 years and over for Aboriginal and Torres Strait Islander people) who:
- are living in the community and have continence issues
- need support to better understand their health conditions
- have a Support at Home package and may be eligible for a dementia supplement.
We prioritise adults from marginalised and/or disadvantaged communities and anyone living in public housing. We also see newly arrived refugees living in Merri-bek.
You need a referral from My Aged Care (MAC) or your Home Care Package case manager to use this service.
To get a referral, either:
- call My Aged Care on 1800 200 422. Tell them you want Holstep Health as your preferred provider
- or ask your case manager to refer you.
