Supporting you to live well through memory changes

If you’re experiencing memory changes or living with dementia, we’re here to help you stay independent and continue doing what matters to you. Our team will work with you and your caregivers to understand what’s happening and offer practical and emotional support tailored to your needs.

We focus on helping you adapt, keep active, and feel supported. You’ll have a team of health professionals including an occupational therapist, physiotherapist, exercise physiologist, nurse, social worker, leisure therapist, dietitian, speech pathologist and a Medicare-funded geriatrician. Our team will do a thorough assessment of your needs and help guide you and your caregivers through any challenges.

We can help you to:

  • adapt your daily activities or environment
  • stay physically active and reduce your risk of falls
  • understand and manage memory changes or dementia
  • find practical ways to respond to behavioural changes 
  • access continence advice and support for funding applications
  • manage your medications and other health conditions 
  • maintain your emotional wellbeing (and that of your carers/family)
  • get started with advance care planning.

Who is this for?

Anyone 65 years and over (50 years for Aboriginal and Torres Strait Islander people), with or without a formal diagnosis of dementia.

What do I need to do?

To use this service, you need a referral from My Aged Care (MAC), who will carry out an assessment.  

You can contact My Aged Care on 1800 200 422 and tell them you want to access Holstep Health – Memory and Dementia Service as your provider. Your doctor or health professional can also refer you using this MAC online form.

A separate referral is needed from your doctor to see the geriatrician – download and complete this form.
