Health and wellbeing programs for Australian First Nations community members

Holstep Health offers a range of programs and services to assist Aboriginal and/or Torres Strait community members with physical health, mental health, social and cultural supports.

We have a strong commitment to enhancing relationships, respect and opportunities with Australia’s First Nations peoples. Our Aboriginal Health Team at West Heidelberg includes both Aborginal and non-Aboriginal staff members.

Our Aboriginal health funded programs for people living in the north east area of Melbourne.

Integrated Team Care Program

  • This is for Aboriginal and/or Torres Strait Islander community members who have chronic health conditions. Our program can help you if you are having difficulty accessing and managing multiple health care services and supports.
  • Our chronic disease nurse and Aboriginal health liaison can work with you to assess you health needs, create a care plan, and help you to access health care providers.

Mental Health and Wellbeing Supports

  • This program offers culturally safe mental health and social supports for Aboriginal and/or Torres Strait Islander Community Members.
  • Our counsellor, social worker and Aboriginal mental health liaison offer culturally specific supports to help improve your mental health and wellbeing. This may include one-on-one support, as well as social and cultural groups

Early Years Support Program

  • This program supports Aboriginal and/or Torres Strait Islander families who may be attending or needing to connect with Maternal and child health services, early years supports or child health services through Holstep Health. Find out more at our Child, Youth & Families services.
  • The Djilak-Djirri Playgroup provides a culturally safe place for families with Aboriginal and/or Torres Strait Islander children to gather, yarn and play.

Aboriginal and/or Torres Strait Islander people in need of these supports who are living or staying in the North Eastern area of Melbourne.

What do I need to do?

Integrated Team Care Program: Ask your GP about a referral if you think you would benefit from this kind of support.

Mental Health and Wellbeing Supports: Contact us on 9450 2000.

Early Years Support Program: Contact us on 9450 2000 or talk to your maternal and child health nurse.
