Since 2022, Holstep Health has led the Social Inclusion Shared Measurement Project. The project aims to support local organisations to work together on improving social inclusion and to measure their progress. It builds on earlier work by the Inner North West Prevention Alliance. That work was led by the Inner North West Primary Care Partnership, which commissioned Trezona Consulting Group to use a co-design approach to create a set of core indicators for tracking and evaluating social inclusion efforts. The resulting Social Inclusion Framework is available below:
Holstep Health supports more than 15 partner organisations to understand and apply the Social Inclusion Framework within their organisation, as well as to collect social inclusion data in a way that enables collective monitoring and evaluation across the region. These collective results are available for partners to view on a secure, password-protected data dashboard hosted by Department of Health and Department of Families, Fairness and Housing.
